Community Health Center of Meriden is seeking a Staff Nurse (RN) to join our primary care team in Meriden, CT. This part-time, per diem role supports a full-range nursing practice for patients across the lifespan, including pre-visit planning and patient education.

The position requires an active CT nursing license and CPR certification, with BSN or ASN preferred. Experience with electronic health records and care coordination is highly valued, and collaboration with clinicians is essential.

Qualifications

• Registered nurse in good standing and CT license.
• Current CT Nursing License.
• American Red Cross CPR for the Professional Rescuer and AED or AHA BLS/AED certification.

Responsabilités

• Support primary care under planned care model, including pre-visit planning and nurse-led visits.
• Collaborate with PCP to coordinate preventive and chronic care.
• Provide health education during visits and calls; monitor self-management goals.
