• Support primary care of patients under planned care model.
• Collaborate with PCP to identify preventive care needs and care coordination.
• Provide general and specific health education to patients and families.
• Administer vaccines and medications under standing orders as applicable.
• Document all patient care accurately in the electronic health record.
• Maintain infection control standards and safe clinical practice.
