Job summary

Southeast Primary Care Partners in Georgia seeks an Advanced Practice Provider (PA/NP) to join a collaborative Family Practice/Internal Medicine team delivering high-quality outpatient care with possible inpatient duties. You will diagnose, treat, and manage chronic conditions in partnership with physicians.

Qualifications

• Graduate of an accredited PA or NP program.
• Current licensure and prescriptive authority through the Georgia Board (or equivalent).

Responsibilities

• Diagnose and manage acute and chronic conditions in outpatient settings (with potential inpatient duties).
• Order and interpret diagnostic tests (labs, imaging, EKGs).
• Prescribe medications and treatments to address conditions.
• Educate patients on conditions, plans, and expected outcomes.
• Collaborate within a multidisciplinary team to deliver evidence-based care.
• Perform additional duties to support patient care and organizational goals.
• Participate in limited on-call duties, primarily phone-based.
