Job summary

Mountaineer Homecare, part of the LHC Group and within the Optum family, is seeking a Physical Therapist Assistant to perform treatment and education per the plan of care. You will work in home health, addressing active and passive exercise, gait, and ADL training to improve patient well-being.

The role requires state licensure to practice physical therapy, a valid driver’s license, and weekend availability. CPR certification is preferred or obtainable within 90 days.

Qualifications

• Licensed to assist in the practice of physical therapy in state of practice.
• Current CPR certification or ability to complete within 90 days of hire.
• Available to work a weekend rotation.

Responsibilities

• Follow state-specific laws governing physical therapy in home care.
• Follow treatment plan defined by supervising PT.
• Instruct patients in active and passive exercise and gait training.
• Coordinate care with healthcare team and report patient responses.
• Provide patient, family, and caregiver education on therapy areas.
