Job summary

UW Health Care Direct seeks an Occupational Therapist to empower home health patients in the Dodge County area to participate in daily tasks. The OT conducts skilled evaluation, planning, and treatment to help patients learn or relearn essential self-care activities at home.

Minimum 0.5 FTE with weekend/holiday coverage possible. Requires Wisconsin OT licensure, a Bachelor's in OT (Master's preferred), CPR/BLS certification, driver’s license, and reliable transportation.

Qualifications

• Bachelor’s degree in occupational therapy required.
• Master’s degree in occupational therapy preferred.
• Wisconsin Driver’s License required.

Responsibilities

• Evaluates and re-evaluates patients to develop and modify effective assessments and treatment plans.
• With patient and family input, establishes goals that reflect anticipated outcomes.
• Provides and progresses therapeutic interventions that are safe, timely, patient-centered, and compliant with policies.
• Promotes self-care and infection control, while maintaining safety and quality in daily work activities.
