Qualifications

• Licensed Occupational Therapist in Wisconsin; BLS certification required within 6 months; valid driver's license.
• Bachelor's degree in Occupational Therapy required.
• Typically 3 years of OT experience in acute hospital, home health care, or related setting.

Responsibilities

• Evaluate patient's physical, cognitive, and functional abilities and impact on daily living.
• Conduct OT evaluation and establish plan of care and treatment.
• Maintain accurate records per state and federal guidelines.
• Plan and implement treatments to retrain and adapt daily living activities.
• Communicate plans and changes to physicians and management.
• Manage caseload, scheduling, and coverage needs.
• Educate patients and caregivers; arrange adaptive equipment as needed.
• May supervise OTAs and ensure compliance with WI regulations.
• Assist with patient transfers and mobility training.
• Ensure patient care adapts to age-specific needs and department policies.
