Job summary

NECCO (ED Necco & Associates Inc.) is seeking a dedicated Foster Care Case Manager to support children and foster families across office, school, and community settings, promoting permanency and safe, stable placements.

You will collaborate with service providers and families, develop and monitor treatment plans, ensure compliance with regulations, and coordinate services while maintaining confidentiality and delivering compassionate care to vulnerable youth.

Responsibilities

• Participate in referral matching for children and families, advocating for the best home match.
• Facilitate and develop each child’s Initial Treatment Plan and Comprehensive Treatment Plan.
• Facilitate and ensure completion of assessments (Biopsychosocial, Independent Living, Diagnostic, etc.).
• Refer foster child to applicable support services (behavioral health, independent living, etc.).
• Develop and implement each consumer’s Individualized Treatment Plan (ITP).
• Assist foster parents and child with implementation and monitoring of the ITP.
• Establish relationships with foster parents and children through training and support.
• Meet state/licensure requirements regarding contact with caseload; ensure medical and therapy services are provided.
• Serve as liaison between Necco and placement agencies and community resources.
• Provide ongoing training and support to foster parents and children.
• Maintain confidentiality and document appropriately; participate in CAP tasks as assigned.
• Deliver services in multiple environments and travel between locations as essential.
