Job summary

Concord Hospital in New Hampshire seeks a Transfer Center Nurse (RN) to serve as the single point of contact for admissions and transfers, coordinating patient placement and bed management. Collaboration with the Medical Director, Nursing Director, and Nurse Manager ensures efficient patient flow.

The role requires a Bachelor’s in Nursing or enrollment within 6 months, NH RN license, BLS, and at least five years in acute care with leadership experience.

Qualifications

• Bachelor’s degree in Nursing or enrolled into a program within 6 months.
• Current and active New Hampshire Registered Nurse license.
• American Heart Association Healthcare Provider BLS required on hire.
• Minimum five years of Acute Care Nursing experience.
• Previous supervisory/ leadership experience desired.
• Demonstrates leadership and management skills in a complex, matrix environment.

Responsibilities

• Coordinate all admissions and transfers via Transfer Center.
• Communicate with referring facilities regarding transfer and bed assignment.
• Monitor and improve quality of care and patient flow.
• Support capacity management and service-line development.
• Engage in daily implementation of strategic goals relevant to capacity management.
• Perform other duties as assigned.
