Job summary

Boston Medical Center is seeking a Transfer Center Nurse (RN) to coordinate admissions and transfers across its health system facilities. The role serves as a liaison among patients, clinicians, and receiving facilities to ensure safe, timely transitions.

Ideal candidates hold a nursing degree (Diploma/ASN/BSN; BSN preferred) with at least 5 years of clinical nursing experience and Massachusetts RN licensure. Per Diem scheduling spans Day, Evening, Nights, and Weekends with EPIC EHR proficiency.

Qualifications

Minimum of 5 years of clinical nursing experience; transfer/admissions experience preferred.
Massachusetts RN licensure required; BSN preferred within four years.

Responsibilities

Coordinate patient admissions from external and internal sources; verify clinical and insurance information.
Manage transfer requests prioritizing medical urgency, bed availability, and resources.
Serve as primary contact for physicians, nurses, case managers, and receiving facilities during admission/transfer.
Review records for accuracy and regulatory compliance; ensure proper bed assignments and status updates.
Provide clinical assessment and triage to ensure safe levels of care during transfer.
