Job summary

Concord Hospital Health System is seeking a Transfer Center Nurse (RN) to serve as the single point of contact for facilities and providers requesting access or patient transfers to Concord Hospital. The role ensures patient placement and flow, balancing bed requests with priority of need, and collaborates with leadership to decide admissions and transfers.

This position requires 5+ years of acute care nursing, leadership experience, strong communication, and flexibility for varied shifts.

Qualifications

• Bachelor’s degree in nursing or active enrollment within 6 months.
• Current and active New Hampshire Registered Nurse license.
• American Heart Association Healthcare Provider BLS required on hire.

Responsibilities

• Anticipates and coordinates admissions and transfers; communicates with referring facilities.
• Monitors and improves quality of care and services provided.
• assesses capacity and patient flow to inform bed management and service planning.
• Executes daily capacity management and supports strategic goals for service lines.
• Supports Health System goals and performs other duties as assigned.
