Boston Medical Center Health System is seeking a Transfer Center RN to coordinate patient admissions and transfers across facilities. You will liaise with patients, physicians, and receiving facilities to ensure safe, timely care.

The role requires strong clinical judgment and excellent communication, along with experience in admission processes and EPIC EHR usage. The position, based in Boston, offers per diem shifts including day, evening, nights, and weekends, and opportunities to support

Formación

• Minimum 5 years of clinical nursing experience.
• Transfer center or admissions department experience preferred.
• Experience in emergency, critical care, or intermediate care preferred.
• Massachusetts RN license or eligible to practice.

Responsabilidades

• Coordinate admissions from external and internal sources with verified information.
• Manage transfer requests based on urgency and bed availability.
• Serve as primary contact for physicians, nurses, case managers, and facilities.
• Review medical records for accuracy, completeness, and compliance.
• Facilitate bed assignments and communicate status updates.
• Provide clinical assessment and triage during admissions/transfers.
• Collaborate with case management and social work on complex cases.
• Maintain accurate admissions/transfers records for QA.
• Participate in initiatives to improve transfer center operations.
• Provide coverage for Off-Shift Nurse Supervisor as needed.
