Job summary

Freeman Health System is seeking a Transfer Center Nurse to support Freeman West and Freeman East operations. This part-time night position focuses on coordinating patient transfers, triaging clinical data, and ensuring timely bed placement.

You will serve as the main contact for referring hospitals and collaborate with nursing leadership to maintain safe, efficient patient flow. Requirements include RN or LPN licensure and BLS, with 2–5 years of acute care experience; BSN preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Active, unencumbered RN or LPN license in the state of practice or a multi-state compact license.
• Basic Life Support (BLS).
• 2-5 years of acute care experience.

Responsibilities

• Transfer Coordination: Process incoming requests for transfers, direct admissions, and consultations from referral sources.
• Clinical Triage & Triage: Assess patient acuity and determine level of care and mode of transportation.
• Bed Management: Collaborate with house supervisors and units to assign beds based on capacity.
• Liaison & Communication: Facilitate physician-to-physician and nurse-to-nurse reports between teams.
• Regulatory Compliance: Ensure transfers adhere to EMTALA, HIPAA, and other standards.
• Documentation: Record all tasks and medical information in EHR and transfer systems.

We work alongside charge nurses and leaders to assign staff and patients to appropriate units to ensure the best patient care
Our Nursing Resources team consists of Bed Control Specialists, Staffing Coordinators and Direct Call Specialists serving Freeman West and Freeman East

What You'll Do

The Transfer Center Nurse facilitates safe, efficient patient transfers by triaging clinical data and coordinating bed placement. They serve as the primary point of contact for external hospitals seeking to refer patients to the health system, ensuring that every transition is medically appropriate and timely.

Requirements

• Education: Graduate of an accredited school of nursing
• Licensure: Active, unencumbered Registered Nurse (RN) license or Licensed Practical Nurse (LPN) license in the state of practice or a multi-state compact license.
• Certifications: Basic Life Support (BLS).
• Experience: Requires 2-5 years of acute care experience.

Preferred Requirements

• Education: Bachelor of Science in Nursing (BSN) is preferred.
• Certifications: Advanced Life Support (ACLS).
• Experience: Preference for backgrounds in Emergency (ER) or Critical Care (ICU).

Responsibilities

• Transfer Coordination: Process incoming requests for transfers, direct admissions, and consultations from various referral sources.
• Clinical Triage & Triage: Utilize clinical expertise to assess patient acuity and urgency, recommending the necessary level of care and mode of transportation (ground or air).
• Bed Management: Collaborate with house supervisors and nursing units to assign beds based on current facility capacity and service capabilities.
• Liaison & Communication: Facilitate physician-to-physician and nurse-to-nurse reports, acting as a "clinical air traffic controller" between sending and receiving teams.
• Regulatory Compliance: Ensure all transfers adhere to EMTALA, HIPAA, and other regulatory standards.
• Documentation: Accurately record all tasks, conversations, and medical information into electronic health records (EHR) and transfer tracking software.
