Job summary

Freeman Health System is seeking a Transfer Center Nurse to coordinate safe patient transfers across Freeman West and Freeman East. You will triage clinical data, coordinate bed placement, and serve as the main point of contact for referring hospitals.

The role requires 2–5 years of acute care experience, and current RN or LPN licensure with BLS. BSN preferred; join a team committed to efficient, high‑quality patient care.

Qualifications

• RN or LPN license in the state of practice or multi-state license.
• BLS certification required.
• 2–5 years of acute care experience.

Responsibilities

• Facilitate safe, efficient patient transfers by triaging clinical data and coordinating bed placement.
• Serve as primary contact for external hospitals referring patients to Freeman Health System.
• Ensure transitions are medically appropriate and timely.
