Job summary

Penn State Health is seeking a Transfer Center Liaison – Patient Logistics to accept incoming referrals, coordinate patient placement across PSH entities, and support Hershey Medical Center. This full-time night shift role requires strong organization and collaboration across clinical teams.

The position offers 0.90 FTE, rotating weekends, and opportunities for professional development within a leading health system.

Qualifications

• Three (3) years of nursing experience required, preferably in Critical Care or Emergency Department.
• Currently licensed to practice as a Registered Nurse by the Pennsylvania Board of Nursing.

Responsibilities

• Accept incoming referral information for PSH entities via multimedia.
• Coordinate and optimize patient flow across PSH hospitals.
• Support patient placement staff at PSH Hershey Medical Center with coordination and prioritization.

SUMMARY OF POSITION:

Responsible for accepting incoming referral information for all PSH entities via multimedia, managing patient placement at PSH community hospitals, as well as collaborating and supporting the patient placement staff at PSH Hershey Medical Center. Coordinates, prioritizes, and optimizes patient flow for the Penn State Health system using knowledge of clinical and operational activities throughout the hospital network.

MINIMUM QUALIFICATION(S):

• Three (3) years Nursing experience required, preferably Critical Care and / or Emergency Department experience
• Currently licensed to practice as a Registered Nurse by the Pennsylvania Board of Nurse Examiners required

PREFERRED QUALIFICATION(S):

• Bachelor's degree in Nursing
• Critical Care and / or Emergency Department nursing experience preferred
