St Johns Community Health in Los Angeles is seeking a Linkage to Care Specialist to perform screenings, needs assessments, and connect clients to services including warm handoffs to PRIME and Behavioral Health.

The role supports grant-funded objectives, conducts intake, and works with a multidisciplinary team to improve HIV prevention outcomes. Evening/weekend availability may be required for community events.

Qualifications

• Minimum 2 years' experience working with transgender individuals in a social service agency or health center setting.
• High School diploma or equivalent required; Bachelor's degree preferred.
• Ability to work effectively with a multi-disciplinary team and excellent communication skills.
• Transgender-affirming and knowledgeable about the population of focus.

Responsabilités

• Provide PrEP, PEP, STI and other risk-reduction education to patients at risk for HIV.
• Act as main point of contact for patients seeking to make initial appointments for HIV prevention services or HIV prevention services.
• Coordinate medication refill assistance with patient and pharmacy communication and referrals.
• Screen patients for appropriate benefits and refer to benefits counselor if necessary.
• Create and maintain program files and records.
• Attend program related meetings for or in place of Program Director.
