Job summary

UC San Diego's Trans Health Project seeks a Case Manager to provide trauma-informed, gender-affirming support for Black and Latina transgender women. You will coordinate care, assist with housing, HIV prevention, and behavioral health needs, and advocate across care systems while maintaining confidentiality.

The role emphasizes culturally responsive practices, Spanish language preference, and collaboration with researchers to support engagement and retention in the THP initiative.

Qualifications

• Bachelor's degree in social work from a CSWE-accredited program; 1 year of experience or equivalent combination.
• Comfort with PC/Windows-based environments; proficient with Microsoft Office.
• Ability to manage multiple tasks with shifting priorities.
• Experience with LGBTQ communities and HIV-affected individuals, especially Black/Latina transgender women.
• Fluency in English; knowledge of inclusive language and respectful terminology.

Responsibilities

• Provide trauma-informed, gender-affirming case management.
• Coordinate care and link services across health, housing, and behavioral health systems.
• Advocate for participant needs and facilitate engagement and retention in THP research activities.
• Collaborate with community partners and research team while maintaining confidentiality.
