Job summary

San Francisco Community Health Center is seeking a Case Manager to provide comprehensive case management for Trans:Thrive clients, coordinating with internal and external providers to improve health outcomes. You will conduct intakes, develop care plans, and offer health education and advocacy.

The role requires experience with transgender communities, harm reduction, and strong communication skills. Some weekend/evening outreach may be required and training will be provided.

Qualifications

  • Bachelor's degree in a health sciences or social work field or equivalent work experience.

Responsibilities

  • Conduct intake and assessments, develop care plans, and coordinate with PCPs, RNs, CHWs and care coordinators.
  • Provide case management and supportive counseling for ECM, Trans Well, STAHR, HOT, and related programs.
  • Provide information and referrals to primary care, behavioral health, dental, vision, public benefits, housing, and monitor referral completion.
  • Deliver HIV/STI health education and linkage support to PrEP, PEP, DoxyPEP, ART, LAI, and HIV/STI testing.
  • Assist with resource navigation during Trans:Thrive drop-in sessions.
  • Participate in meetings, trainings, and agency events; maintain documentation within 24 hours.
  • Support QA processes, peer review, and clinical consultations.
  • Perform other duties; occasional weekend and late-night outreach as needed.

Since 2007, SFCHC’s Tran:Thrive has provided comprehensive medical and social services for the Trans/GNC community of San Francisco. More than a drop-in center, Trans:Thrive is a safe space for community members to access programming to improve their quality of life. Offering case management, medical services (including Primary Care, HRT, referrals for gender affirming surgery), substance use services and treatment (including harm reduction supplies, counseling, treatment linkage), support groups, meals, and community events.

Role Summary:
The Case Manager of Trans Services is primarily focused on providing case management services to eligible trans/GNC clients a part of Trans:Thrive programs and the SF Community Health Center. The fundamental tasks to perform are, but not limited to, the following: conducting client intakes and assessments, developing care plans, and providing care coordination with other internal and external providers including PCPs, RNs, CHWs and care coordinators. The Case Manager is also responsible for providing as needed health education, supportive counseling, crisis intervention, benefits advocacy, and coordination of services within the agency and between other agencies.

  • Conduct intake and assessment to eligible clients, provide care plans and supportive meetings to complete goals per individual care plans.
  • Provide case management and supportive counseling for clients engaged in case management programs including, but not limited to Enhanced Care Management (ECM), Trans Well, San Francisco Transgender Alliance for Health Resources (STAHR), House Of Thrive (HOT), etc.
  • Provide information and referrals to services including to primary care, behavioral health, dental, vision, public benefits, managed care plans, shelter and housing, overdose prevention, and employment as well as monitor referral completion
  • Provide as needed HIV/STI health education and linkage support to PrEP, PEP, DoxyPEP, ART, LAI, and HIV/STI testing.
  • Support with providing as needed resource navigation to clients during Trans:Thrive drop-in.
  • Participate in case conferences, department meetings, monthly staff meetings, in-service training, and agency-wide retreats and outreach events.
  • Complete necessary documentation, assessments, care plans, and encounters in a timely manner, preferably within 24-hours.
  • Participate and comply with agency quality assurance, peer review, and clinical consultation
  • Perform other duties and tasks as needed.
  • This position requires working occasional weekend and late-night outreach community events on behalf of the agency to engage with targeted populations

Qualifications:

  • BA/BS in a health sciences or social work-related field or AAA/CMH/CHWORcomparable work experience.
  • One year of experience providing case management, counseling, advocacy, health education, service coordination, and/or outreach work to disenfranchised communities.
  • At least one year of experience working, or interpersonal experience with transgender individuals and issues.
  • Experience working with diverse populations including trans, BIPOC, the LGBQ communities, refugees/immigrants, people living with HIV/AIDS, people experiencing homelessness, people who are currently or formerly incarcerated, and dual-diagnosed populations (substance use and/or behavioral health issues).
  • Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques and trauma informed care.
  • Knowledge of HIV, STIs, substance use, mental health, social service and probation systems.
  • Must be adaptive, flexible and a team player.
  • Strong verbal and written proficiency in English.
  • Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills.
  • Familiarity and experience with Microsoft Word and Outlook programs
  • Detail-oriented with strong follow-through skills, including attention to detail.
  • Must be able to sit, stand, and walk for extended periods of time.
  • Fully vaccinated against COVID-19, complete with booster shot, and able to show proof of vaccination.
  • Up to date with vaccinations, including seasonal flu shot vaccine and TB clearance.

Preferred:

  • Bicultural, bilingual in Spanish (or other SF threshold language)
  • Experience working in a Federally Qualified Health Center (FQHC) or other type of community health centers.
  • Self-starter and self-motivated.
  • PC computer skills preferred, especially with MS Office products
  • Experience in providing social services or refugee assistance
  • Knowledge of EPIC, Excel, Zoom and being able to learn other computer programs related to administrative work.
