Job summary

UC San Diego Health's Trans Health Project (THP) seeks a case manager to provide comprehensive, trauma-informed, gender-affirming case management to Black and Latina transgender women in San Diego, addressing health care access, housing, HIV prevention, and other social determinants.

Under supervision, the role supports participant engagement, coordinates care with community partners, ensures confidentiality, and prioritizes culturally responsive services; Spanish-language proficiency is

Qualifications

Bachelor's degree in social work and one year of experience; or an equivalent combination of education and experience.
Comfortable working in PC/Windows-based environments and proficient with Microsoft Office.
Ability to manage multiple tasks with shifting priorities.
Experience working with LGBTQ communities and HIV-affected populations in San Diego County.
Fluency in English; competence in working with diverse populations.
Knowledge of person-first language and inclusive language used with minority populations.

Responsibilities

Provide trauma-informed, gender-affirming case management to Black and Latina transgender women.
Support access to health care, HIV prevention/treatment, housing, economic security, and other social determinants.
Coordinate care, link to services, advocate with systems, and ensure confidentiality and research compliance.
