Job summary

UCAN (Chicago) seeks a TPSN Transition Coordinator to bridge services for pregnant and parenting youth involved with DCFS. You will lead the C21 initiative, coordinate across agencies, and ensure smooth administrative processes while guiding clinical teams.

In this role, you will manage relationships with DCFS and TPSN, supervise interns, and support case management with timely documentation, data reporting, and service coordination to improve youth outcomes.

Qualifications

• Master’s degree in social work or related field.
• 2–3 years’ experience with pregnant/parenting clients or child welfare.
• LCSW or LCPC preferred.
• Driver’s license required.

Responsibilities

• Serve as liaison to DCFS and TPSN on the C21 project and related tasks.
• Lead TPSN C21 project coordination, data management and reporting.
• Supervise interns and provide reflective supervision to staff.
• Monitor project data and report to external stakeholders.
• Support case management with clinical consultation and crisis guidance.
