UCAN Chicago’s Teen Parenting Service Network seeks a TPSN Transition Coordinator in Chicago, IL. The role acts as liaison to agencies serving pregnant and parenting youth in DCFS care, coordinating C21 processes, and supervising interns as needed.

You will lead projects, manage data, provide clinical consultation, and build relationships with DCFS and community partners. A Master’s degree in social work or related field and Illinois driver’s license required.

Qualifications

• Master’s degree in social work or related field.
• 2–3 years’ experience with pregnant and parenting clients or child welfare.
• LCSW or LCPC preferred.
• Car and valid Illinois driver’s license required.

Responsibilities

• Lead TPSN C21 project, coordinating DCFS relations and project logistics.
• Monitor and manage data for projects, reporting to stakeholders.
• Provide clinical consultation and supervision to staff and interns.
