Seamar is seeking an Integration Specialist for Transitions of Care to ensure health care continuity for at-risk populations in Tacoma, WA. This role advocates for patients and their families, providing education and support during transitions from hospitals to home.

The ideal candidate will have a background in human services, strong communication skills, and the ability to work effectively with diverse populations. A passion for community health and patient-centered care is essential.

Qualifications

• Experience working with underserved populations.
• Ability to navigate healthcare systems.
• Bilingual in English and Spanish preferred.

Responsibilities

• Ensure continuity of care during patient transitions.
• Educate patients and caregivers on health management.
• Conduct outreach to eligible patients within two business days post-discharge.
