Qualifications

• BSW or BA/BS in Human Services, Health Sciences or related field.
• Experience working with underserved, transient populations.
• Bilingual Spanish/English preferred.

Responsibilities

• Support patient self-management by enhancing health literacy and goal setting.
• Advocate to secure appropriate patient services and navigate care.
• Educate patients and families using teach‑back methods.
• Coordinate cross‑setting communication between primary and specialty care.
• Document activities and contribute to monthly reports.
