Job summary

Navajo County in Arizona is seeking a public health outreach worker to develop and provide education, conduct outreach, and connect community members to services. This grant-funded position is contingent on the availability of funds.

Under supervision, you will deliver evidenced-based sessions on topics like adolescent health, substance use prevention, naloxone, chronic disease prevention, and emergency preparedness; establish partnerships with schools and healthcare providers; maintain records

Qualifications

• High School diploma or GED is required.
• Three years’ experience in health field with one year in general office work.
• Valid driver’s license required.

Responsibilities

• Conduct outreach to individuals, families, schools, and community organizations to promote healthy behaviors and program participation.
• Deliver evidenced-based and culturally appropriate health education sessions.
• Assist in planning, coordinating, and evaluating program activities and events.
• Provide support and referrals to healthcare and social services as appropriate.
• Establish and maintain cooperative relationships with schools, healthcare providers, community partners, and other stakeholders.
• Maintain accurate records of outreach, education sessions, and client interactions; prepare required reports.
• Identify barriers to care and advocate for improved access to health and wellness services.
• Participate in trainings to stay current with public health best practices.
• Perform other related duties as assigned.

Under general supervision, performs work of moderate difficulty in developing and providing public health education, conducting outreach, and connecting community members to services; performs related work as assigned. This is a grant funded position, and dependent upon the availability of grant funds.

• Conducts outreach to individuals, families, schools, and community organizations to promote healthy behaviors and program participation.
