Lutheran Services in Iowa is seeking a Thrive Resource Navigator to support families in Thrive Iowa by providing assessment, advocacy, and resource navigation. You will coordinate services, facilitate care teams, and document outcomes to promote stability and self-sufficiency.

Qualifications

• Required: Associate’s degree and relevant experience in human services, family support services, case management, or equivalent.
• Preferred: Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or related field.
• Ability to assess needs, coordinate services, and advocate for families.

Responsibilities

• Provide assessment, support, advocacy, and resource navigation for Thrive Iowa participants.
• Coordinate services and facilitate care teams for assigned families.
• Complete documentation and follow-up within required timeframes.
• Respond to referrals promptly and maintain accurate case notes.
• Collaborate with community providers, schools, health systems, and public assistance programs.
