Job summary

Kaleidoscope is seeking a Full-Time Community Resource Manager - THRIVE to connect families with resources and coordinate referrals in the Chicagoland area. The role emphasizes equitable access, strong partnerships, and data-driven program support.

Based in Illinois, the position requires travel, a Master's degree in social work or related field, and experience in community-based service delivery. Hybrid work and evening/weekend hours may be required.

Qualifications

• What you’ll need is a Master’s degree in social work or related field; MSW preferred.
• MSW: 1 year of social service experience; Master’s degree: 2 years of social service experience
• LCSW or LCPC preferred but not required; CWEL/CANS/CERAP Illinois certifications desired.

Responsibilities

• Coordinate THRIVE project activities, including scheduling meetings, agendas, and minutes.
• Build and strengthen partnerships with community organizations and service providers.
• Identify gaps in services and improve collaboration among providers.
• Attend coalitions and partner meetings to advocate for family needs.
• Maintain a resource directory organized by social determinants of health.
• Develop community engagement activities and events.
