Job summary

Lutheran Services in Iowa is seeking a Thrive Resource Navigator to work with families in Thrive Iowa, providing assessment, support, advocacy, and resource navigation to improve stability and access to services.

You will coordinate with schools, healthcare providers, and community partners; complete documentation in HopeHub; and support families with a strengths-based, family-centered, and culturally responsive approach. Bilingual candidates are encouraged.

Qualifications

• Bachelor's degree or higher in social work, human services, psychology, counseling, sociology, education, criminal justice, public health, or related field.
• Associate’s degree with relevant experience in human services, family support services, case management, community-based services, or equivalent combination of education and experience as approved by HR.

Responsibilities

• Provide assessment, support, advocacy, resource navigation, service coordination, care team facilitation, documentation, and follow-up for Thrive Iowa participants.
• Coordinate with community providers, schools, health care systems, workforce organizations, child welfare professionals, public assistance systems and other partners.
• Use HopeHub with fidelity, complete closed-loop referrals, and follow required timelines and reassessments every 30 days.
• Maintain strengths-based, family-centered and culturally responsive approach and ensure accurate records.
