Job summary

Jobtailor seeks a dedicated Developmental Disabilities Waiver Coordinator to perform in-home assessments and video consultations from the corporate office in Albuquerque. You will maintain care records, document contacts, and ensure HIPAA compliance while delivering respectful, person-centered support.

This role requires strong organizational skills, the ability to travel across the Albuquerque metro area, and excellent communication with participants, families, and partner agencies.
