MedCadre is seeking a skilled home health professional for day shifts to support a range of patients in home settings. The role emphasizes assessment, documentation, and patient-family education during visits.

Responsibilities include 6–8 home visits daily, weekend coverage, and using BayCare tools for scheduling and timesheet entries. Requires 1 year of experience, valid driver’s license, and vaccination status as specified.

Formación

• One year of home health experience required.
• Valid driver’s license and access to a car for home visits.
• COVID vaccination or approved exemption on file.

Responsabilidades

• Perform home skilled visits, typically 6–8 per day.
• Provide patient education and ensure accurate documentation.
• Assist with postoperative and chronic condition care in home settings.

Home skilled visits

Home skilled visits - 6-8 per day (MINIMUM of 6 points per day at managers discretion may increase by 1 point)

Weekends

1-2 Weekends per contract

Other requirements

1 year experience required

Must have valid drivers license and car to make home visits.

Must be COVID vaccinated or with exemption form on file.

Must have flu shot or declination and in both cases mandatory mask wearing at all times while at work.
