MedCadre in Augusta, GA is seeking a Physical Therapist for a Home Health geriatrics role. Shift is Day, 8:00–16:30, with 7 visits and a full-time productivity target around 25 units. Requires a GA PT license or Compact PT and BLS; minimum 1 year of experience; EPIC transition planned for Sep 2026.
Mileage is reimbursed at IRS rates, and Home Health Base (HCHB) experience is preferred. Scrub color is flexible; RTO up to 7 days; holidays may be required.

Ratio / Census: 7 visits; Full Time Productivity is around 25 units.
Unit Systems: Home Care Home Base (HCHB) highly preferred.
Transitioning to EPIC: around September 2026.
HCHB Course: If the clinician doesn't have experience with Homecare Homebase (HCHB), they will be required to complete an online HCHB course prior to starting. The course is *** and must be paid by traveler upfront (margined).

Holidays: If one major holiday falls in his/her assignment period, the traveler is expected to work it. If two major holidays fall in his/her assignment period, the traveler is expected to work both of the holidays. Compliance: Full compliance required to start on time; if not compliant in time the push back is 2 weeks due to bi-weekly orientation schedule.
CLINICAL SCREEN: Candidate will be reviewed by clinical to determine clinical fit, if clinical pass and within Piedmont guidelines, candidate will be sent to the manager for final decision. Clinical screens will be sent on case-by-case basis.
