Job summary

Cynet Health is seeking a Physical Therapist (Therapy/Rehabilitation) for home health in South Carolina. The role covers multiple counties with a typical daily travel, and you will group patients to minimize drive time.

Description:The therapist will provide services in a designated area covering multiple counties.The majority of patients are located in specific counties.The scheduler will work to group patients together on specific days to minimize travel time.The typical daily travel is anticipated to be approximately 20-30 miles, with an occasional maximum of 70 miles.Mileage will be compensated at the standard rate and tracked accordingly.Experience in home health care is required.The patient population will include adults receiving rehabilitation for various conditions such as joint replacements, COPD, CHF, and Type 2 Diabetes Mellitus.Therapists will be expected to maintain a productivity standard that involves an average number of visits per day.Candidates will be subject to a clinical screening to assess their fit within the organization.Compliance with all onboarding requirements is mandatory before beginning work.Flexibility to float within the designated service area and to cover multiple patient needs may be necessary.Major holidays may require work commitments as determined by the assigned schedule.Adaptability in documentation practices related to home health services is required.Overall, the position demands a proactive approach to patient care, emphasizing efficiency and effectiveness in service delivery.
