Atrium Health is seeking a licensed Doctor of Physical Therapy to manage a patient caseload in home health. You will assess, plan, and implement PT services in collaboration with the treating physician and multidisciplinary team.
Responsibilities include data collection, documentation, and coordination of care across settings, ensuring high-quality outcomes. Travel to patient homes is required, with adherence to safety and regulatory standards.
Doctor of Physical Therapy (DPT) degree required. This aligns with current professional entry standards, as the DPT has been the recognized entry-level credential for all new graduates since 2016; prior bachelor’s or master’s-level degrees may be accepted only where consistent with applicable licensure requirements. Maintains continuing education requirements as mandated by applicable state professional standards.
Physical Therapy License in the State of applicable state or South Carolina must be licensed in the state upon which you work. Basic Life Support for Healthcare Provider certification from the American Heart Association required. Current state driver's license and auto insurance at all times. Personal vehicles must be well maintained and in safe driving conditions.

Demonstrates knowledge of principles of growth and development and demonstrates the skills and competency appropriate to the ages, culture, developmental stages, and special needs of the patient.
Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling. Must be able to lift and support the weight of 50 pounds in handling patients, medical equipment, and supplies. Ability to travel extensively during all seasons to assigned patient's place of residence. Works in a variety of home environments and conditions. Must speak English in good understandable terms. Intact sense of sight, hearing, smell and touch. Finger dexterity. Critical thinking and the ability to concentrate. Must be able to respond quickly to changes in patient and/or unit conditions. Ability to work flexible schedule to meet patient's needs. Personal Protective Equipment such as gloves, goggles, gowns, and masks are sometimes required due to possible exposure to hazardous chemicals or blood and body fluids.
1-year experience as a licensed PT in a clinical care setting or home health preferred.
Pediatric setting – 1 year pediatric experience within the past 5 years preferred or education and training may be provided by Advocate Health At Home.
