Overview
We are seeking a skilled and qualified Primary Care Physical Therapist to service our patients throughout the Midlands East area. This is a salary-based, full-time 8-hour position (8:00 AM – 5:00 PM), Monday–Friday, performing visits in homes, ALFs, and ILFs, clinics, and via Telehealth. There is an opportunity to receive a monthly bonus.
Responsibilities
- Provide high-quality physical therapy services to patients in multiple settings (homes, care facilities, clinics, and telehealth).
- Obtain complete health and psychosocial histories and records findings in a systematic format.
- Perform comprehensive physical assessments and provide health education and counseling.
- Arrange referrals to other health team members as needed and to community resources.
- Use appropriate reporting mechanisms as required by law; respect patient dignity, confidentiality, and privacy; obtain necessary authorizations and consents for treatments.
- Review patient medical diagnoses, prognosis, procedures, and clinical course; participate in community education and outreach.
- Market and provide community education as part of care delivery.
- Travel between offices, facilities, and other sites as required.
Qualifications
- Successful completion of a Physical Therapy program and licensure to practice in the state of South Carolina.
- Current CPR certification.
- Prior healthcare experience is preferred.
- Ability to work flexible hours and travel between locations.
- Ability to utilize electronic documentation.
- At least one year of experience in the specialty field (Pelvic Health experience preferred).
