Responsibilities

• Lead the day-to-day clinical presence of the Prava therapy program within the community.
• Maintain an active clinical caseload while mentoring the therapy team.
• Establish a culture of clinical excellence, accountability, collaboration, and resident-centered care.
• Orient and integrate new clinicians into the community, residents, culture, routines, and expectations.
• Coach and mentor therapists in clinical reasoning, documentation, communication, and functional outcomes.
• Build relationships with community leadership, nursing, care partners, residents, and families.
• Identify residents who may benefit from therapy through interdisciplinary conversations.
• Ensure therapy goals reflect residents' daily activities and needs.
• Support residents with mobility, neurologic, cognitive, dementia, and functional status needs.
• Partner with caregivers to reinforce strategies for safe, confident daily function.
• Monitor clinical quality, documentation, outcomes, scheduling, and caseload trends.
