Job summary

Health Connect America seeks a licensed or license-eligible therapist to provide high-quality mental health services to clients across multiple states. Qualifications

• Master’s degree in Social Work, Counseling, or a related field is required.
• Must be eligible for licensure as a LPC, LMFT, or LCSW in the applicable states.
• Experience with biopsychosocial assessments and treatment planning is expected.

Responsibilities

• Perform comprehensive biopsychosocial assessments to determine needs and interventions.
• Create individualized, measurable treatment plans with clients and families.
• Deliver individual, group, and family therapy sessions.
• Document all client interactions, treatment plans, and progress notes per policy.
• Coordinate with a multidisciplinary treatment team for holistic care.
• Engage in outreach to promote services and attract referrals.
• Provide crisis intervention and develop safety plans when needed.
