Job summary

Health Connect America is seeking a Therapist to provide high-quality mental health services in an on-site setting. You will conduct assessments, develop treatment plans, and deliver individual, group, and family therapy while collaborating with multidisciplinary teams.

The role requires a Master’s Degree in Social Work, Counseling, or a related field and eligibility for licensure as LPC, LMFT, or LCSW in applicable states.

Qualifications

• State-specific requirements for licensure in multiple states (LPC/LMFT/LCSW) across TN, NC, MS, AL, SC, VA.
• Master’s Degree in Social Work, Counseling, or a related field required.
• LICENSURE eligibility required for applicable state licensure.

Responsibilities

• Perform biopsychosocial assessments to determine needs and interventions.
• Develop individualized treatment plans with clients and treatment teams.
• Deliver individual, group, and family therapy sessions.
• Maintain accurate documentation and progress notes within policy timelines.
• Engage in outreach to promote services and referrals.
• Coordinate with care teams and emergency services when needed.
