Job summary

Community Health Resources seeks a Therapist for Pathways OTP in Enfield, CT. This full-time 40-hour/week position serves clients through MAT and counseling in a multi-setting program, with supervision and ongoing professional development.

The role requires a Master’s degree and licensure eligibility, with trauma and SUD experience preferred. The program emphasizes comprehensive treatment planning, crisis assessment, and family and service-provider collaboration.

Qualifications

• Master’s degree or higher in a related field is required.
• Experience working with trauma, mental illness and/or SUD preferred.
• Licensure: LMSW, LPC-A, LMFT-A required; others may be substituted.

Responsibilities

• Provide assessment and treatment interventions per program model.
• Conduct clinical and risk assessments.
• Collaborate to create individualized, person-centered treatment plans.
• Demonstrate competency with diverse client populations and needs.
• Develop discharge plans utilizing natural supports and community resources.
• Maintain client and staff records per policy.
• Provide coverage during absences and participate in supervision.
