CHR Community Health Resources, Inc. in Enfield is seeking a full-time Therapist for Pathways OTP, Mon-Fri, 5:30AM-2:30PM, 40 hours/week.

You will assess, develop individualized treatment plans, and coordinate care across office, home, and community settings, with supervision and training support.

Master’s degree and Licensure (LMSW/LPC-A/LMFT-A) required; driver’s license often needed; CHR offers competitive benefits and professional growth.

Qualifications

• Master’s degree or higher in related field.
• Experience with trauma, mental illness and/or substance use disorders preferred.
• Licensure: LMSW, LPC-A, LMFT-A required; other licenses may be accepted in some programs.

Responsibilities

• Provides assessment and treatment interventions per program model.
• Conducts clinical and risk assessments.
• Develops individualized, person-centered treatment plans.
