Job summary

Axis Health System is seeking a Jail Transitions Therapist to support patients from incarceration into the community. You will design care plans, provide therapy, and coordinate referrals to prevent recidivism across La Plata and Montezuma counties.

This role involves collaboration with multiple programs and occasional travel, with opportunities to work in outpatient settings and across Axis locations in Colorado.

Qualifications

• Bachelor's degree in a recognized mental health profession.
• Minimum 2 years’ experience in mental health.
• Colorado License Eligible or Licensed LPC/LMFT/LCSW/PsyD in Colorado.

Responsibilities

• Design and implement individual care plans for program clients.
• Provide therapeutic interventions, referrals, linkage and advocacy to prevent recidivism.
• Collaborate with jail and community programs for full integration into community.
• Travel between Axis locations as required.
