Job summary

Axis Health System in Durango, CO seeks a Jail Transitions Therapist to support patients incarcerated and to continue services in the community. This role designs and implements individualized care plans, provides therapy, referrals, and advocacy to prevent recidivism, coordinating with community programs for seamless reintegration.

The position covers jail and outpatient settings, requires a bachelor’s in a mental health field and at least two years of experience; Colorado license eligibility

Qualifications

• Bachelor’s degree in a recognized mental health profession with at least 2 years’ experience.
• Master’s degree preferred; Colorado License Eligible/Licensed LPC, LMFT, LCSW or PsyD in Colorado preferred.

Responsibilities

• Participate as a member of the Jail Transitions program team to design and implement the individual care plans for program clients.
• Provide therapeutic interventions, referral, linkage, and advocacy for program patients to prevent recidivism.
• Collaborate with other community programs to ensure full integration into the community for the patient.
• When not working with jail population, provide general outpatient treatment.
• Utilize a variety of treatment approaches and case coordination to work with a diverse client population.
