Qualifications

• Master's degree in Social Work with LMSW, LCSW, LPC or PLPC required.
• Strong knowledge of behavioral health levels of care, insurance authorization processes, and patient intake workflows.
• Excellent communication, documentation, and customer-service skills.

Responsibilities

• Serve as primary intake and referral inquiry contact, ensuring documentation and follow-up.
• Maintain relationships with referral sources, patients, families, and clinical teams.
• Review clinical information to assist level-of-care determinations.
• Schedule face-to-face level-of-care assessments within 24 hours of initial contact.
• Communicate services, admission criteria, regulatory requirements, and payer considerations.
• Facilitate referrals to community-based or alternate levels of care when not appropriate.
• Guide patients through admission, obtaining consents and documentation.
• Obtain insurance pre-certifications and collaborate with billing on payer requirements.
• Track and provide weekly referral reports to leadership.
• Collect patient and physician feedback for quality improvement.
• Participate in performance improvement activities.
