Job summary

Adelante Healthcare Roosevelt in Phoenix, AZ is seeking a Therapist I to deliver behavioral health services in both individual and group settings. The role includes initial intakes, diagnostic assessments, and developing treatment plans within a multidisciplinary primary care model.

The therapist will work with psychiatrists, case managers, and medical assistants, using evidence-based therapies and coordinating referrals to ensure comprehensive patient care.

Qualifications

• Master's degree in Behavioral Health (Psychology, Social Work).
• Computer literacy required.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide behavioral health services to children, adults, and families as part of the continuum of care.
• Complete initial intake and diagnostic assessments for all clients in the Behavioral Health Program.
• Develop initial treatment plans and update plans with observable objectives and goals.
• Provide clinical consultation to providers and staff in mental and behavioral health.
• Assess DSM-V/ICD-10 diagnostics as referred by Adelante Medical staff.
• Coordinate care and referrals to outside sources or back to PCP after treatment goals.
