Job summary

Fallon Health's Summit ElderCare PACE program seeks a compassionate activities coordinator to plan and lead therapeutic recreation for older adults. You will conduct group and individual activities, tailor plans to cognitive, physical, and social needs, and collaborate with an interdisciplinary team.

Valid CPR/Alzheimer's certifications or willingness to obtain them, reliable transportation, and prior health-care activity program experience are preferred.

Qualifications

• High school graduate or equivalent.
• One or more years of experience in an activities program in a health care setting serving frail or elderly population within the last 5 years, or completion of Fallonhealth offered training program.
• CPR and Alzheimer's certifications, or willingness to be certified.
• Access to reliable transportation to travel throughout the service area to perform essential functions.

Responsibilities

• Conduces a variety of large and small therapeutic recreational groups and individual activities tailored to improve or maintain participants’ cognitive, physical, social, psychosocial, spiritual and artistic needs.
• Contributes activity ideas for monthly planning calendars.
• Demonstrates programming for educational and culturally competent activities including exercise, arts and crafts, music appreciation, and spirituality.
• Ensures participant choices and engagement when planning activities.
• Builds a warm social atmosphere and supports participants at the ADHC.
