Job summary

Fallon Health’s Summit ElderCare is seeking an Activities Assistant to support the interdisciplinary PACE team in Dartmouth, MA. The role focuses on therapeutic recreation for elderly participants, planning individual and group activities, and fostering social engagement within a caring team.

The position requires a high school diploma, CPR/Alzheimer’s certification (or willingness to obtain), and reliable transportation to cover the service area.

Qualifications

• High school diploma or equivalent is required.
• CPR and Alzheimer’s certifications, or willingness to be certified, are essential.
• Access to reliable transportation to facilitate travel throughout the service area.

Responsibilities

• Conducts therapeutic recreational groups and individual activities to address cognitive, physical, social, and psychosocial needs.
• Plans and documents activity assessments and participation for PACE participants.
• Fosters a warm, respectful environment and supports participants’ independence and engagement.
• Participates in Interdisciplinary Team meetings and care conferences as assigned.
• Maintains equipment and supplies and reports any safety concerns.
• Ensures attendance and participation are documented daily per PACE requirements.
