Job summary

Fallon Health’s Summit ElderCare in Leominster, MA seeks an Activities Assistant to join the PACE interdisciplinary team. The role centers on assessing participant needs and developing individualized recreation plans for an aging population.

You will plan, organize, and deliver activities promoting physical, cognitive, and social well-being while maintaining documentation, safety, and quality of care in a supportive, respectful environment.

Qualifications

• High school diploma or equivalent required.
• CPR certification preferred or willingness to be certified.
• Alzheimer's certification preferred or willingness to obtain.

Responsibilities

• Conducts therapeutic recreational groups and individual activities to address cognitive, physical, social, and spiritual needs.
• Contributes ideas for monthly activity planning calendars.
• Ensures participant engagement and culturally competent activities.
• Maintains cleanliness and safety of recreational supplies and equipment.
• Documents attendance and participation daily for records.
