Qualifications

• Experience providing strengths-based clinical coaching to individuals with IDD and co-occurring mental health needs.
• Ability to lead teams, supervise staff, and ensure program fidelity.
• Experience coordinating crisis prevention and post-discharge support.

Responsibilities

• Provide strengths-based clinical coaching to a small caseload.
• Conduct initial therapeutic assessments and develop person-centered plans.
• Implement crisis prevention plans in collaboration with team.
• Deliver post-discharge supports for individuals returning home.
• Collaborate with families and caregivers to support sustainable strategies.
