Responsibilities

• Lead clinical coordination and support for a caseload of individuals with IDD and dual diagnoses.
• Develop and implement CSCPIPs using strengths-based approaches.
• Collaborate with families, caregivers, and external providers for integrated care planning.
• Provide 24/7 on-call crisis response and post-crisis debriefing.
