Job summary

JEVS Human Services is seeking a START Coordinator to join a clinical team providing community-based assessment, crisis prevention, planning, and therapeutic coordination to stabilize individuals in crisis.

You will lead person-centered planning, build cross-system collaboration, and apply trauma-informed care within an integrated service model, making a lasting difference for individuals and families while growing professionally in a supportive environment.

Responsibilities

• Clinical Coordination & Support: Manage a caseload of 20-30 individuals and develop CSCPIPs using strengths-based, trauma-informed approaches.
• Systems Integration & Community Collaboration: Serve as a clinical liaison across behavioral health, developmental services, medical, educational, and emergency services.
• Crisis Response & On-Call Support: Share in the 24/7 on-call rotation, provide timely telephonic and face-to-face crisis response.
• Documentation & Compliance: Ensure timely documentation in SIRS and EMR systems; track outcomes and support admissions, discharges, and transitions.
• Professional Growth & Training: Participate in internal trainings, CEUs, and clinical conferences; maintain required certifications.
