Qualifications
- Requires a Master's degree in Behavioral Health or Social Work or nursing degree with 2–4 years of related experience.
- Licensure required: LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN.
Responsibilities
- Evaluates post-discharge needs and conducts post-discharge assessments for members transitioning from healthcare facilities.
- Performs medication reconciliation between pre-admit and post-discharge medications.
- Develops care/service plans and collaborates with discharge planners, providers, specialists, and teams to support member transition and discharge needs.
- Assesses health status, resource needs, and treatment plans to provide appropriate interventions.
- Facilitates transition to active care management and educates members and caregivers to promote wellness.
