Job summary

Centene Corporation seeks a care management professional to assess, plan, and coordinate post-discharge services for members in Texas. You will perform medication reconciliation, collaborate with discharge planners and providers, and support transitions across the continuum of care.

You will educate members and caregivers, document all activities for compliance, and contribute to cost-effective quality care. This role offers flexible work arrangements and opportunities to impact community health.

Qualifications

• Requires a Master's degree in Behavioral Health or Social Work or Nursing degree with 2–4 years of related experience.
• Licensed social worker (LISW/LCSW/LMSW/LMFT/LMHC/LIC/LP C) or RN required.
• Experience with post-discharge care management and medication reconciliation preferred.

Responsibilities

• Evaluates post-discharge needs and transitions from healthcare facilities.
• Performs medication reconciliation between pre-admit and post-discharge meds.
• Develops care/service plans with discharge planners, providers, and interdisciplinary teams.
• Assesses member health status, resources, services, and treatment plans; provides interventions.
• Facilitates transition to active care management according to member needs.
• Educates members and caregivers on wellness activities and resources.
