AccentCare Inc. is seeking a Clerical Territory Patient Admissions Coordinator in Sacramento, onsite, with a salary of $23.00 - $28.00 per hour and a Monday–Friday, 8am–5pm schedule. Medical backoffice experience is required for consideration.

The role involves coordinating with clients, families, and referral sources, partnering with agency staff, and ensuring timely, quality referrals through EMR systems and payor communications.

Qualifications

• High School diploma or GED.
• Minimum of one year experience in home care, hospice, or personal care services.
• Prior use of Electronic Medical Records (EMR), HCHB preferred.
• Experience dealing with Medicare/Medicaid/payors preferred.

Responsibilities

• Collaborates with Referral Source, Sales teams, Clients and/or local agency operations to collect information for a quality referral.
• Documents referral information, actions taken, and follow-up in the medical record in a timely and accurate manner.
• Ensures timely attention to referrals from platforms such as eFax, phone, or portal.
• Reviews Medicare eligibility alerts and responses when applicable.
• Requests necessary documents for ICD 10 coding by the CPOCC team.
• Performs follow-up calls to confirm information and gather missing demographic and physician details.

What You Need to Know

The Territory Patient Admission Coordinator establishes and maintains communication with the clients, family members and referral sources, in a given territory. This role is responsible for partnering with agency staff, referral sources, and sales to achieve optimal patient satisfaction & outcomes. The Territory Patient Admission Coordinator is responsible for the timely acceptance or decline of the referral based off the predetermined criteria set by the Agency and facilitates obtaining all information and documentation necessary to complete a quality referral. This position’s assigned territory, may encompass one or more states in AccentCare’s operation and may change with the needs of the business.

• High School or GED

• Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred

Responsibilities

• Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
• Documents all referral related Information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency’s software solution to facilitate the referral process by completing required workflow task within the Company's established time frames.
• Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
• Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
• Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.
• Completes a “Follow Up” call to the Client/Caregiver/Referral Source to collect or confirm information. Example: confirms out of pocket expense, clarify teachable caregiver, and collect missing demographic and physician information.
